Abstract The Affordable Care Act (ACA) ushered in some of the largest changes to U.S. healthcare policy over the past few decades. Because of the changes in taxes and subsidies and the creation of the Health Insurance Exchanges, the small group market, which generally insures employers with less than fifty employees, is the sector where the ACA was perhaps most expected to disrupt existing health insurance. The purpose of this paper is to understand the interplay between employer-sponsored insurance for the small group market and Health Insurance Exchanges. Specifically, we evaluate who, in terms of individual characteristics and health risk, benefitted from being able to purchase insurance in the small group market relative to having this market eliminated and being faced with exchange plans. We then consider the implications of individual preferences as to adverse selection in the small group market. Finally, we consider whether counterfactual exchanges, for instance, with larger subsidies or tax deductibility of plans, or more generous plan benefits, would have affected the market differently.
